Reporting an Incident or an Issue

We welcome players, parents, coaches, and visiting teams to complete this incident/issue report to have your issue heard by the Duluth Amateur Hockey Associaton's Board of Directors.

1. Complete this report which will immediately be sent to DAHA upon completion. 

2. The board will review your report and the appropriate committee will contact you to set up either a phone call or meeting.

Your complaint, incident, or issue will NOT be heard if you decide to show up to the DAHA office unannounced or call the DAHA office. Please take the time to fill out a report so we have time to understand what is happening and reach out with a time to meet or talk. 

DAHA's mission is to provide a safe and positive environment for community members. Also, to give kids an opportunity to learn important life lessons and develop their hockey skills. We will work our hardest to make sure this is the case for the entire Duluth Community.
